Circumstances:
The crew and one of their friend decided to leave Ayanka that day for a fishing party by the Penzhina River located 8 km north of Ayanka. En route, while cruising at a too low altitude, the airplane struck tree tops, stalled and crashed in flames on a sandbank. The pilot and the passenger were killed while another pilot was seriously injured. The crew was not authorized to perform such a flight.

On a flight from Milkovo to Khalaktyrka in support of a geological expedition, the crew encountered below-minima weather conditions (visibility limited to 10 km and snowfall). The crew failed to return and deviated from the prescribed flight path to the left by 17 km. The aircraft crashed at a height of 589 metres into the cloud-covered summit of a hill (594 metres) located in Mys Nalycheva, some 45 km northeast of Khalaktyrka. The wreckage was found a day later and all three occupants were killed.
Probable cause:
It was determined that the crew decided to continue the flight despite the deterioration of the weather conditions. The failure of ATC to ask the crew to return to Milkovo or to divert to another airport was considered as a contributing factor, as well as the fact that meteorologists failed to inform the crew about the deterioration of the weather conditions.

Two hours after his departure from Magadan on a flight to Petropavlovsk-Kamchatsky, while cruising by night at an altitude of 3,600 meters, the crew informed ATC that the right engine failed. The pilot was instructed to divert to Sobolevo Airport for a safe landing but he preferred to continue to the destination despite the fact the aircraft was losing altitude as the power on one engine was insufficient. The aircraft descended to 2,700 meters and the pilot was aware of the mountain en route (2,000 meters high) and estimated his altitude was sufficient (gap of about 600 meters). Few minutes later, weather conditions deteriorated with snow showers and the visibility became very limited. Henceforth at an altitude of 2,000 meters in poor visibility, the airplane struck the slope of Mt Yurshik (2,059 meters high) located about 60 km northwest of Petropavlovsk-Kamchatsky. The wreckage was found three days later, on January 4, and all 23 occupants have been killed.
Probable cause:
In-flight failure of the right engine due to the failure of the cylinder head. The decision of the crew to continue to Petropavlovsk-Kamchatsky instead of diverting to the nearest airport was inappropriate.

On a flight from Ossora along the coastline to Anapka (Ilpyrski), the crew encountered below-minima weather conditions (heavy snowfall combined with poor visibility). The single engine aircraft deviated from the prescribed flight path by 7.5 km due to strong winds and crashed at a height of 500 metres into the slope of a mountain located 11 km northeast of Rybozavod (fish factory) No. 72 in the Ossora district. Two passengers were killed and eight other occupants were injured. Two of the survivors managed to walk to the fish factory within 6 hours of the accident. Others occupants were rescued on January 3, 1965 due to poor weather conditions.
Probable cause:
Information related to weather conditions and transmitted to the crew were inaccurate and failed to reflect the truth. Also, the crew decided to continue the flight into below-minima visibility.

The aircraft departed Ust-Bolsheretsk Airfield at 1520LT bound for Kikhshik, carrying five passengers, a crew of two, 117 kilos of mail, 176 kilos of various goods, among them 58 bottles of alcool and 300 kilos of luggage. After takeoff, the crew was instructed to return to Ust-Bolsheretsk in case they encounter snow en route. Few minutes into the flight, weather conditions deteriorated with snow falls and clouds down to 300 meters when the captain decided to continue the flight to Kikhshik at an insufficient altitude of 100 meters (minimum safe altitude fixed at 250 meters on this route). Approaching Kikhshik Airfield, the pilot-in-command attempted to land and made a steep turn to the right when control was lost. The airplane struck the ground 570 meters on the left of the airfield, bounced and came to rest 70 meters farther. All seven occupants were injured and the aircraft was destroyed. Two days later, one of the passenger died from his injuries. The captain was seriously injured.
Probable cause:
The crew neglected the ATC instructions and decided to continue the flight under VFR mode in IMC conditions. The approach to Kikhshik was completed below minima with an insufficient visibility of 1,000 meters and the pilot made a turn to the right at an excessive angle of 30° and at a too low altitude to expect recovery. Poor weather conditions at destination was considered as a contributing factor.

The aircraft was performing a cargo flight from Milkovo to Khalaktyrka, carrying a crew of four and a load of 46 bags of potatoes for a total weight of 2,536 kilos. The crew started the descent in clouds and did not realize he was not following the assigned route for Khalaktyrka. At an altitude of 2,100 meters, the airplane struck the north face of Mt Aag (2,319 meters high) located 42 km north of Khalaktyrka. The wreckage was found a day later and all four crew members were killed.
Probable cause:
It was determined that strong winds were blowing at 60 km/h at the time of the accident, causing the airplane to deviate 12° from the prescribed route. Due to clouds, the crew was unable to distinguish the mountain. Lack of ATC assistance was considered as a contributing factor.

The crew was performing the flight Ust-Khayryuzovo - Palana with intermediate stops at Tiguil and Voyampolka, carrying three passengers, two pilots and 954 kilos of deer meat carcasses. Shortly after takeoff from Voyampolka, the copilot told the captain they forgot to deliver a package so the pilot decided to return, completed a low pass at a height of 50 meters, opened his window and jettisoned the package when the aircraft banked left and lost altitude, causing the left wing to struck the ground. Out of control, the airplane cartwheeled and crashed upside down on an icy terrain. The airplane was destroyed and all five occupants were injured. One of the passenger died from his injuries few hours later.
Probable cause:
The accident was the consequence of a careless take of risk on part of the pilot who attempted to drop a package in flight while it would be more prudent to land. Doing so, he failed to pay attention to the conduct of the flight and the airplane attitude.

The single engine aircraft left Milkovo Airport at 1235LT on a back trip to Petropavlovsk-Kamchatsky with eight passengers and a crew of three on board. The crew continued the flight under VFR at an altitude estimated between 1,200 and 1,300 meters. About an hour into the flight, weather conditions deteriorated with low clouds and rain falls when the aircraft hit the slope of a mountain located about 18 km northwest of Koryak. As the airplane failed to arrive in Petropavlovsk-Kamchatsky Airport, SAR operations were conducted and the wreckage was found three days later in an isolated area. All 11 occupants have been killed.
Probable cause:
Failure of the flying crew to adhere to the published and prescribed procedures while flying under VFR in adverse weather conditions.

The airplane departed Ust-Bolsheretsk on a flight to Tilichiki, carrying three crew members and two people from the Soviet Communist Party. After a flight of two hours and 30 minutes, the airplane landed in Ust-Kamchatsk where the crew passed the overnight. At the airport, the crew consumed alcohol and failed to sleep enough time. The next day, on March 12, the airplane took off at noon for Tilichiki despite the pilot was not fit. At 1241LT, the radio operator informed ground that he passed Ozernaya at an altitude of 1,500 metres. Half an hour later, while cruising in good weather conditions, the airplane entered an uncontrolled descent and crashed in the taiga about 22 km north of the village of Uka. The wreckage was found the next day and all five occupants were killed.
Probable cause:
It is believed that the pilot may fall asleep or lost consciousness during flight, probably due to a lack of sleep and/or intoxication.

During the day, the pilot already performed twice the flight between Petropavlovsk-Kamchatsky and Ust-Bolsheretsk. After passing over Apacha, weather conditions deteriorated and the visibility dropped to zero due to heavy snow falls. Nevertheless, the pilot attempted to land at Ust-Bolsheretsk and continued the approach when he lost control of the airplane that crashed six km east of the destination airfield. The airplane was destroyed and the pilot was killed.
Probable cause:
The decision of the pilot to initiate an approach in below-minima weather conditions. Lack of discipline on part of the pilot and his failure to return in time were considered as contributing factors.
