Shortly after takeoff, the single engine airplane went out of control and crashed, bursting into flames. Both pilots were seriously injured and both passengers, an American couple, were killed.

After landing at Kalongo Airstrip, the twin engine airplane went out of control and came to rest upside down with its right wing torn off. Both pilots escaped uninjured.

Shortly after takeoff from Kampala-Kajjansi Airport, the crew encountered technical issues and elected to return for an emergency landing. Upon touchdown, the single engine airplane went out of control, lost its left wing and engine before coming to rest upside down in a grassy area. Both pilots were rescued, the copilot was slightly injured and the captain was seriously injured. The crew departed Kajjansi Airfield on a positioning flight to Mweya to pick up passengers to Entebbe.

The airplane departed Kinshasa on cargo flight to Lisala, carrying one passenger, four crew members and a load consisting of medical supplies against Malaria. On approach to Lisala Airfield, weather conditions were poor with strong winds and heavy rain falls. The airplane landed at a speed of 118 knots some 433 metres past the runway 05 threshold (runway 05 is 2,200 metres long). After touchdown, the crew initiated the braking procedure but the airplane was unable to stop within the remaining distance. It overran, lost its nose gear and came to stop in a field 172 metres past the runway end. All five occupants evacuated safely and the airplane was damaged beyond repair.

The pilot encountered an unexpected situation and elected to make an emergency landing in an open muddy field. He lowered the landing gear and upon touchdown, the airplane overturned and came to rest upside down. All five occupants were rescued and the airplane was destroyed.

After liftoff from Nasir Airfield, the single engine airplane encountered difficulties to gain height. It rolled to the right, impacted terrain and crashed in an open field, bursting into flames. All occupants evacuated but an elderly passenger later died from injuries sustained. The airplane was totally destroyed by a post crash fire.

Shortly after takeoff from Juba Airport runway 31, while climbing, the crew encountered an unexpected situation and was forced to attempt an emergency landing. The airplane crash landed in an area located in the Hai Referendum district about 3 km northwest of the airport. All five occupants escaped with minor injuries while the airplane was severely damaged.

After landing at Mweya Airstrip located in the Queen Elizabeth National Park, the single engine airplane was unable to stop within the remaining distance. It overran and collided with a house. All five occupants escaped uninjured and the aircraft was damaged beyond repair. It just completed a charter flight with three European citizens and two pilots on board.

During the approach, the crew encountered heavy rain and thunderstorms with lightning as well as significant crosswind and turbulence. During the final approach, several nose down inputs on the control column increased the descent rate of the aircraft. The EGPWS SINK RATE-SINK RATE alert triggered 15 seconds before the accident. Then the EGPWS PULL-UP warning triggered, followed by flight crew's nose up input on control column, around 2 seconds before the aircraft struck the water surface of Lake Victoria about 500 meters short of runway 31 threshold. The impact with the water caused significant damage to the aircraft fuselage followed by water entering the cabin and flight deck. There was no fire but the aircraft was damaged beyond economic repair. Of the 43 persons on board, 17 passengers and 2 pilots lost their lives while 22 passengers and 2 cabin crew members survived without serious injuries. It is considered that conducting an unstabilized final approach in poor weather conditions that could not allow clear visual contact with the ground was a major factor in this accident.The decision of the Pilot in Command to negotiate his way through the narrow weather window in order to reach the runway,the high crew workload caused by the presence of thunderstorms, the variable cross wind, rain and turbulence as well as the absence of air traffic services at Bukoba airport were contributory factors. The aircraft sank by a depth about 3-4 metres.

The airplane departed Kasese Airport at 1500LT on a cargo flight to Goma, carrying two pilots, one passenger and a load of various goods for a total weight of one ton. En route, weather conditions deteriorated with low clouds and heavy rain falls. As the airplane failed to arrive at destination, SAR operations were initiated and the wreckage was found two days later in a dense wooded area located in South Kivu, about 139 km southwest of Goma. The airplane was destroyed by a post impact fire and all three occupants were killed.
