Circumstances:
The crew was supposed to fly to an unknown airfield. As he was unable to find that airfield, the pilot (M. I. Burin) decided to land at Vyazma Airport. He failed to fly the full aerodrome circuit and approached at an angle that allowed the airplane to touch down 120 metres from the perimeter of the airfield. Out of control, it collided with a ditch and ended up in a ravine. There were no casualties.
Probable cause:
Crew error.

Circumstances:
The airplane was engaged in a transfer of wounded soldiers between two airports in the region of Vyazma. While flying at low height, it was shot down by a German fighter and crashed in a field, bursting into flames. All three crew members were injured and the passenger was killed. The airplane was destroyed by a post crash fire.

Circumstances:
The airplane was en route from Belarus to Smolensk, carrying one passenger, five crew members and a load of brochures that should be distributed in Smolensk. Approaching Smolensk from the southeast, the airplane was mistakenly shot down by the Soviet Flak (ground artillery). The pilot reduced his altitude and attempted an emergency landing. The airplane crash landed in a rough terrain and came to rest. All six occupants escaped uninjured and the airplane was damaged beyond repair.
