Circumstances:
The crew was completing a training mission from Hiroshima to Hōfu. While approaching Hōfu Airport, the four engine aircraft struck the slope of Mt Ohira located 8 km northeast of the airfield. All three crew members were killed.

Shortly after takeoff from Iwakuni AFB, while climbing to an altitude of 300 feet, the left engine caught fire. The crew lost control of the airplane that crashed into the sea about 1,500 meters offshore. Both pilots were injured while the aircraft sank and was lost.

During a night landing on rough seas off Iwakuni AFB, the seaplane went out of control and came to rest upside down. Four crew members were killed while seven others were rescued. It was determined that the visibility was low at the time the crew tried to land on rough seas with high winds.
