The single engine airplane departed Nabire Airport at 1254LT on a schedule service to Kaimana, carrying 11 passengers and two pilots. Two minutes after takeoff, while climbing, the crew informed ATC about a loss of engine power and was cleared for an immediate return and landing on runway 17. Unable to do so, the pilot attempted an emergency landing and ditched the airplane into the sea, few hundred metres northwest of the airfield. All 13 occupants were rescued.

The airplane departed Yogyakarta-Adisutjipto Airport on a flight to Makassar (Ujung Pandang), carrying 10 people. While descending to Makassar-Sultan Hasanuddin Airport Runway 21, it crashed against Mt Bulusaraung (5,135 feet high) located 26 km northeast of the airport. The airplane disintegrated on impact and all 10 occupants were killed. The wreckage was found at an altitude of 5,100 feet. At the time of the accident, the airplane was not on the correct approach path for a landing on runway 21.

After landing on runway 25 at Ilaga-Aminggaru Airport, the single engine airplane was unable to stop within the remaining distance. It overran by few metres, veered to the right and collided with a wooden guard post before coming to rest against the perimeter fence, bursting into flames. Both crew members, a pilot and a technician, escaped with minor injuries before the airplane would be totally destroyed by fire. The crew completed a cargo flight from Timika-Moses Kilangin Airport.

The twin engine airplane departed Gorontalo-Jalaluddin Airport at 0702LT on a schedule service to Panua Pohuwato, in the southwest part of the Gorontalo Province. On final approach to Panua Pohuwato Airfield, the airplane lost height and crashed in a marshy field located near the airport. The airplane was destroyed and all four occupants were killed.

During the takeoff roll from Serui-Stevanus Rumbewas Airport Runway 28, the crew decided to abort for unknown reasons. The airplane veered off runway to the left. While contacting soft ground, it lost its undercarriage and slid for few dozen metres before coming to rest nearby a wooded area. All 48 occupants evacuated safely. The airplane landed at 1019LT from Biak and was taking off for its last leg to Jayapura when the accident occurred.

The single engine airplane departed Tarakan on a cargo service to Binuang, carrying two crew members and a load of 583 kilos of food supplies. En route, the airplane collided with trees and crashed on the slope of a wooded mountain located in the Long Liku area. Rescue teams arrive on site the following day. The pilot was injured and the flight engineer was killed. The airplane was destroyed.

The single engine airplane departed Elelim Airport at 1054LT on a flight to Poik, carrying four passengers, two pilots and a load of cargo. About six minutes later, it impacted the slope of a wooded mountain located 12 km from the airport. The airplane was totally destroyed by impact forces and all six occupants were killed.

The airplane departed Jakarta-Halim Perdanakusuma Airport on a charter flight to Maleo, carrying four passengers and four crew members. Following an uneventful flight at FL390, the crew started the descent to Maleo-Morowali Airport Runway 23. After touchdown, the airplane was unable to stop within the remaining distance, overran, rolled for about 200 metres and came to rest against a wooded hill. All eight occupants evacuated safely, among them four Chinese passengers. The crew consisted of two pilots, one stewardess and one technician. Runway 05/23 at Maleo Airport is 1,000 metres long. It is believed that the airplane was operated by Lionair Charter Division.

The single engine aircraft departed Dekai-Nop Goliat Airport for a short cargo flight to the Dagi Baru Airstrip with two pilots on board. Weather conditions were considered as good upon arrival. After landing, the aircraft went out of control, veered off runway and came to rest down a ravine. Both occupants were injured and the aircraft was destroyed.

The single engine airplane was on an unscheduled cargo flight from Moses Kilangin Airport, Timika (WAYY) to Ilaga Airport in Puncak district of Papua, Indonesia (WAYL). On board in this flight were two pilots and the flight carried 2,352 lbs (1,067 kg) of cargo. The flight departed from Moses Kilangin Airport, Timika on 24 October 2021 at 2154 UTC (25 October 2021 at 0654 LT) on day light. The Pilot in Command (PIC) acted the Pilot Flying (PF) and the Second in Command (SIC) acted the Pilot Monitoring (PM). The flight until commenced the approach at Ilaga was uneventful. According to the witness that was on the Ilaga apron, stated that the short final area of the Runway 25 partially covered by ground fog. Two aircraft that were making approach landing before PK-SNN cancelled the approach and made go around. About 0725 LT, PK-SNN made approach and impacted to the embankment about 15 meters before the runway. The aircraft stopped about 90 meters from the beginning runway 25. A pilot was killed and a second was injured.
