Shortly after takeoff, while in initial climb, the aircraft suffered an engine failure. The crew attempted an emergency landing when the aircraft stalled and crashed in an open field, bursting into flames. All 12 occupants escaped uninjured while the aircraft was destroyed by a post crash fire.

Probable cause:

Engine failure for unknown reasons.

Circumstances:

The aircraft departed Makhachkala Airport at 1829LT and climbed over the sea. Less than two minutes later, the crew was cleared to climb to 2,100 meters when the pilot reported to ATC the presence of clouds over the sea that were not mentioned in the weather bulletin transmitted to him prior to the flight. Suddenly, the aircraft nosed down and plunged into the sea about 3,5 km offshore. The wreckage was found in a depth of 10 meters and all 17 occupants have been killed.

Probable cause:

It was determined that, at an altitude of 400 meters while climbing, the airplane struck a flock of migratory birds that struck the windshield that broke instantly, killing both pilots. It was reported that many birds were migrating in this region, at any time during this period of the year.

En route from Astrakhan to Chkalovsk, the crew encountered below minima weather conditions due to snowfalls. In poor visibility, the pilot decided to divert to Sol-Iletsk Airport but on final, the airplane impacted a snow covered hill located between Boyevaya Gora and Saratovka. All occupants survived the crash but nine of them froze to death within few hours.

Circumstances:

The airplane departed Myachkovo Airport on a cargo flight to Tbilisi with intermediate stops in Borisoglebsk, Astrakhan and Baku. On board were four crew members, one passenger and a load consisting of 365 kilos of mail and various goods. The crew departed Borisoglebsk at 0834LT on the leg to Astrakhan. At 0920LT, while in normal cruise, the airplane was shot down by a German fighter and crashed in the frozen river Medveditsa, near the village of Mironichev. All four occupants were killed.

The airplane departed Kuybyshev on a flight to Baku with intermediate stops in Saratov and Astrakhan. Shortly after takeoff from Saratov Airport, while in initial climb, both engines lost power, causing the airplane to stall and to crash. All 13 occupants were rescued, among them five were seriously injured.

Probable cause:

Loss of engine power caused by a mismanagement of the fuel supply system on part of the crew. It was determined that at the time of the accident, both engines were feeded by the main left tank that was almost empty while sufficient fuel remained on other tanks.
