Circumstances:
The airplane departed Horn Island on a flight to Merauke (Augustus Field), Papua, carrying four crew members. While about 30 km from the destination, flying over a convoy at an altitude of 1,200 feet, the right engine failed. The crew continued the flight on one engine but due to the presence of several vehicles on the runway, including a water truck, the landing was made halfway down on the last 500 metres. The airplane was unable to stop within the remaining distance, overran and crashed in a pile of debris, causing the undercarriage to collapse. All four crew members escaped unhurt and the airplane was considered as damaged beyond repair.

Circumstances:
The crew departed Wagga Wagga Airport (Forest Hill) on a navigational and strip landing exercise. A first stop was performed at Nowra Airport then the crew was supposed to continue to Mallacoota via Moruya before returning to base at Wagga Wagga. The airplane departed Nowra at 0801LT for Mallacoota. It was seen passing over Moruya at about 0830LT, flying at a height of about 50 feet in an easterly direction. It was due to arrive at Mallacoota at about 0900LT but failed to arrive at destination. It is believed that the airplane crashed into the Pacific Ocean somewhere between Moruya and Mallacoota. Lost without trace.

While practicing low level torpedo attack exercise, the pilot was apparently forced to make an evasive action when he lost control of the airplane that crashed into the sea 16 km off Jervis Bay. Three crew members were killed and one was seriously injured.
