Circumstances:
The engine failed in flight, forcing the crew to attempt an emergency landing. The airplane crash landed in a field and was damaged beyond repair. There were no casualties.
Probable cause:
Engine failure in flight caused by a fuel exhaustion.

Circumstances:
The aircraft was engaged in a cargo flight from Ust-Kut to Kirensk, carrying a crew of three, two technicians of the company and two people in charge of the 1,200 kg of cargo. The crew failed to prepare the flight properly, expedited taxi and departure. The airplane took off from runway 30 at a speed of 155 km/h and with an angle of attack of 15°. After rotation, while in initial climb at a height of 120 metres at a speed of 210 km/h, the pilot raised the flaps when the airplane lost height and descended until it struck trees and crashed in flames 1,500 metres past the runway end. The copilot was seriously injured while all six other occupants were killed. Few hours later, the copilot died from his injuries.
Probable cause:
It was determined that the crew precipitated the takeoff procedure and failed to follow the pre-departure checklist. The following contributing factors were reported:
- Poor flight preparation,
- The cabin was not heated prior to departure,
- The left altimeter was erroneously set with a value 10 mm Hg higher than prescribed (wrong altimeter setting),
- Stabilizers were set at -6° instead of the required -3,6° angle,
- The rotation was completed prematurely,
- During initial climb, flaps were raised at a speed of 210-220 km/h, 20 km/h below the speed mentioned in the Flight Manual,
- Lack of visibility,
- Lack of ground visual references.

Shortly after takeoff, during initial climb, the airplane stalled and crashed by the runway. The aircraft was destroyed and all five crew members were injured.
Probable cause:
Loss of control after rotation as the cargo shifted to the rear part of the cabin.

Circumstances:
The aircraft departed Ust-Kut Airport on a cargo flight to Mirny, carrying a load of 18 tons of cement. Few minutes after takeoff, the crew was informed that a landing at Mirny Airport was impossible due to technical problems and the captain decided, without referring to his operations, to divert to Lensk Airport where the weather conditions were considered as poor. At that time, the sky was overcast with clouds down to 600 meters, an horizontal visibility of 2,500 meters with rain and snow. The flight was continued at an altitude of 8,000 meters then the crew was cleared to descent to 3,000 meters and to start the approach to Lensk. On final, the pilot-in-command failed to realize his altitude was insufficient when the airplane struck tall trees (20 meters high) in a flat attitude, continued the descent and crashed 420 meters further in a wooded area. The wreckage was found 15,6 km short of runway threshold. The aircraft was totally destroyed and all six crew members were killed.
Probable cause:
The accident was the consequence of a premature descent and a controlled flight into terrain for undetermined reason. Nevertheless, the assumption that an engine may have failed on approach was not ruled out. At the time of the accident, Lensk Airport was not equipped with an ILS system or any kind of ground guidance assistance system.

Crash of a Lisunov LI-2T in Mama: 8 killed

Circumstances:
The crew was performing a cargo flight to Mama, carrying a load of fish. While descending to a height of 1,500 meters, the crew received the permission to complete the final approach visually when the airplane hit tree tops and crashed in a wooded area located 18 km north of the airport. All eight crew members were killed. At the time of the accident, weather conditions were marginal with low clouds, snow falls, turbulences and icing conditions.
Probable cause:
It was determined that the visibility was insufficient at the time of the accident and the pilot in command descended too low without any visual contact with the obstacles. Ground assistance on part of the ATC was not efficient, which was considered as a contributory factor.

Circumstances:
The airplane departed Irkutsk on a flight to Yakutsk with intermediate stops in Balagansk and Ust-Kut, carrying three passengers, two crew members, 240 kg of bagages and 727 kg of mail. Approaching Ust-Kut, the engine lost power and started vibrating. The pilot shut it down and attempted an emergency landing when the airplane clipped trees and crashed in a wooded area located 50 km south of Ust-Kut. The captain was killed while four other occupants were injured. The following day, they walked through the taïga and found rescue.
Probable cause:
It was determined that the engine failed in flight due to a fuel issue that was caused either by:
- the fuel valve was closed;
- a foreign object obstructed the fuel line;
