Circumstances:
The floatplane departed Angle Bay at noon on a maritime patrol flight over an area located some 100 miles west of Brest. In the late evening, the crew was supposed to return to base. While cruising at an altitude of 1,000 feet, the crew was unable to locate Angle Bay due to the lack of visibility caused by the logical blackout. Short of fuel, the crew attempted to ditch the airplane that impacted the water surface and crashed in the Irish Sea. Six crew members were killed and five others took place in a dinghy and were later rescued.

While returning to base the following night, it collided with a balloon cable over Birmingham and was abandoned by the crew. It is believed that the Hampden, which was set on autopilot following the collision, continued until it crashed in the Irish Sea. All four crew members were uninjured.

The airplane departed RAF Leeming at 1800LT On September 30 to bomb the Air Ministry building in the Leipzigstrasse in Berlin. On return, while flying over England at night, the crew sent a series of distress calls and descended to the Irish Sea where the airplane was ditched at 0640LT. Late in the evening of October 1st, all five crew members were rescued by a trawler with various injuries.
