Circumstances:
The crew departed Santiago on a cargo flight to Arica. While cruising over the Andes Mountains, the aircraft disappeared. As it failed to arrive at destination, SAR operations were initiated but eventually suspended after few days as no trace of the aircraft nor the four crew was found. Walkers found the wreckage in February 1941 on the slope of Mt Mercedario, in the west part of the Argentinian Andes.

While descending to Lima-Limatambo Airport, the crew encountered poor visibility due to fog. Too low, the single engine airplane impacted the slope of a mountain located near Atocongo, some 30 km southeast of the airport. The aircraft disintegrated on impact and all eight occupants were killed, among them two Chilean citizens and the American pilot Stanley Harvey.

While descending to Tacna Airport on a flight from Arica, the pilot encounted limited visibility due to foggy conditions. The aircraft impacted the slope of a mountain and crashed near the village of Palca, about 45 km northeast of Tacna Airport. All four occupants were killed.
