The airplane was carrying 12 wounded soldiers and five crew members when it was attacked by enemy fire. The crew made an emergency landing in a prairie. Later, the crew took off again but the airplane suffered from icing conditions and crashed. There were no casualties.

Circumstances:
Shortly after takeoff from Vyazma-Dvoyevka Airport in the late evening, while in initial climb, the aircraft encountered serious difficulties to gain height, stalled and crashed in a field located 150 metres past the runway end. Upon impact, the passenger was thrown out and killed while all six crew members were injured. The airplane was outbound to Moscow-Vnukovo Airport.
Probable cause:
It was determined that the total weight of the airplane at the time of the accident was 11,600 kg, 1,114 kg above the MTOW. The runway length of 700 metres was considered as too short in such conditions.

Circumstances:
The crew was supposed to fly to an unknown airfield. As he was unable to find that airfield, the pilot (M. I. Burin) decided to land at Vyazma Airport. He failed to fly the full aerodrome circuit and approached at an angle that allowed the airplane to touch down 120 metres from the perimeter of the airfield. Out of control, it collided with a ditch and ended up in a ravine. There were no casualties.
Probable cause:
Crew error.
