Circumstances:
The airplane departed Jerez-La Parra Airport at 1456LT on a flight to Cologne-Bonn Airport with four people on board. It continued at an assigned altitude of 36,000 feet until it entered the German Airspace. German ATC was unable to establish a radio contact with the crew so the decision was taken to send a Panavia Tornado of the Luftwaffe that departed Rostock-Laage AFB and intercepted the Cessna at 1815LT. The military pilot did not see any one in the cockpit and evacuated the area five minutes later. The airplane overflew Germany then entered the Swedish Airspace and continued bound to the northeast without significant change in heading, altitude or speed (365 knots). At 2028LT, the airplane started to descent and initiated a turn to the right three minutes later. At 2040LT, it entered an uncontrolled descent to the left and spiraled to the sea before crashing at 2044LT about 37 km northwest of Ventspils. Few debris and oil were found at the point of impact. The accident was not survivable.
Probable cause:
Cabin pressurization issue suspected.

After a 30-minute uneventful instrument flight rules (IFR) flight, the business jet landed in the rain on the 4,311ft-long runway. The pilot reported, and runway skid marks corroborated, that the airplane touched down about 1,000 ft from the approach end of the runway. The pilot reported braking action was initially normal and the anti-skid system cycled twice before it stopped working and he was unable to slow the airplane using the emergency brakes. The airplane continued off the departure end of the runway where it traveled through wet grass and a fence before coming to rest with the landing gear collapsed. A video of the airplane during the landing roll indicated there was a significant amount of water on the runway. No mechanical anomalies were found with the brake/antiskid systems during the postaccident examination of the airplane. Marks on the runway indicated functionality of the antiskid system. Stopping performance calculations estimated the distance required to stop the airplane on the runway was about 4,127 ft. The runway length remaining after the airplane touched down was about 3,311 ft. The pilot was aware of the runway length and weather conditions prior to departure and reported that he should have not accepted the trip.
Probable cause:
The pilot’s decision to land on a runway that did not provide enough length to stop the airplane given the wet surface conditions, resulting in a runway excursion.

Circumstances:
The Cessna 551 Citation II/SP corporate jet took off at 13:30 local time from Reichelsheim Airfield, Germany, for a training flight at Siegerland Airport. It was a training flight to acquire the type rating for the aircraft. The right pilot's seat was occupied by the pilot in command, who was deployed on this flight as a flight instructor. The student pilot, as co-pilot, sat on the left pilot seat and was the pilot flying. For the co-pilot it was the second flight day of his practical training program on the Cessna 551 Citation II/SP. The day before, he had already completed about three flying hours on the plane. At Siegerland Airport, three precision approaches to runway 31 were carried out with the help of the Instrument Landing System (ILS). After the third landing, the tower, due to the changed wind, turned the landing direction to runway 13. The cockpit crew therefore rolled the aircraft to the end of the runway, turned and took off at 14:34 from runway 13. This was followed by a left-hand circuit at an altitude of 3,500 ft AMSL. The approach to runway 13 took place under visual flight conditions. According to both pilots, the checklists were processed during the circuit and the aircraft was prepared for landing on runway 13. In the final approach, the landing configuration was then established and the landing checklist performed. The copilot reported that shortly before the landing the speed decreased, the aircraft flew too low and the approach angle had to be corrected. He pushed the engine thrust levers forward to the stop. The pilot in command supported this action by also pushing the engine thrust levers forward with his hand. However, according to the pilot in command, the remaining time to touch down on the runway was no longer sufficient for the engines to accelerate to maximum speed in order to deliver the corresponding thrust. He also described that the aircraft had been in the stall area at that time. However, he had not noticed a stall warning. At 14:42, with the landing gear extended, the aircraft touched down in the grass in front of the asphalt area of runway 13. The left main landing gear buckled and damaged the tank of the left wing. The right main landing gear also buckled, the tank on the right side remained undamaged. The kerosene escaping from the left wing ignited and a fire broke out. The aircraft burned and slipped along runway 13 on the folded landing gear, the underside of the airframe and the extended landing flaps until it came to a standstill after a distance of approx. 730 m from runway threshold 13. After the plane had come to a standstill on the runway, the copilot noticed flames on the left side of the plane. The pilot switched off both engines. Then both pilots left the plane via the emergency exit door on the right side. The pilots were not injured.
Probable cause:
The accident, during which the airplane touched down ahead of the runway, was caused by an unstabilized approach and the non-initiation of a go-around procedure.
The following factors contributed to the accident:
- The organisation of the traffic pattern was performed too close to the airport.
- The final approach was flown too short and conducted in a way that it resulted in an unstabilized approach.
- During the final approach the approach angle was not correctly maintained until the runway threshold.
- During the final approach speed was too low.
- Both pilots did not recognize the decrease in speed early enough and had not increased engine performance in time.
- The flight instructor intervened too late and thus control of the flight attitude of the aircraft was not regained soon enough.
- The ascending terrain ahead of the runway threshold was also a contributory factor. It is highly likely that the student pilot had the impression of being too high and deliberately maintained a shallow approach angle.

Circumstances:
The twin-engine corporate jet impacted terrain while maneuvering to land after a global positioning system (GPS) approach. The pilot reported that the airplane entered icing conditions during the approach and that the airplane descended out of instrument meteorological conditions between 300-400 feet above ground level (agl). The pilot reported that his windshield had become obscured by ice accumulation during the approach and that he "had difficulty seeing the runway." The pilot elected to land the airplane instead of executing the published missed-approach procedure. The airplane impacted terrain 439 feet short of the runway threshold while in a right turn. After the accident, there was ice accumulation on all booted airframe surfaces measuring 2-4 inches wide and 1/4 to 3/8 inch thick. The upper portions of the windscreens were contaminated with ice measuring about 3/8 inch thick. The remaining airframe portions, including the heated surfaces, were free of ice accumulation. The windshield bleed air switch was selected on "High" with the pilot's side windshield heat control knob approximately mid-range. Windshield alcohol was selected "On", but the alcohol reservoir was still full upon inspection. At the time of the accident, there was an overcast ceiling of 500 feet agl, 1-3/4 statute mile visibility with mist, and an outside temperature of -08 degrees Celsius. The published minimum descent altitude (MDA) for the GPS runway 17 approach is 500 feet agl, for an airplane equipped with a lateral navigation only GPS receiver. The pilot held a private pilot certificate with multi-engine land, instrument airplane, and Cessna 500 type rating. The pilot reported having 2,200 hours total flight time and 475 hours in the same make/model as the accident airplane.
Probable cause:
The pilot's decision to continue below the minimum descent altitude (MDA) and his failure to fly the published missed-approach procedure. A factor to the accident was the pilot's improper use of windshield heat which resulted in the windshield becoming obscured with ice during the instrument approach in icing conditions.

Circumstances:
The aircraft departed Lins Airport on a ferry flight to Sorocaba with two pilots on one passenger (the owner) on board. The aircraft was transferred to Sorocaba Airport for maintenance purposes. While descending, the crew was informed that runway 36 was in use and that three small aircraft were completing local training in the circuit. In good weather conditions, the captain decided to complete a straight-in approach to runway 18. After touchdown, the aircraft was unable to stop within the remaining distance. It overran, crossed a road and came to rest into a ravine. The passenger escaped uninjured, the copilot was seriously injured and the captain was killed. The aircraft was destroyed.
Probable cause:
Wrong approach configuration on part of the crew who completed an unstabilized approach and landed too far down the runway (about a half way down) at an excessive speed. In such conditions, the aircraft could not be stopped within the remaining distance. The following contributing factors were identified:
- The crew did not make any approach briefing,
- The crew failed to follow the approach checklist,
- The aircraft had deficiencies in maintenance, particularly with regard to the brakes systems,
- The techlogs were out of date,
- Maintenance was periodic but insufficient,
- Although the runway 36 was in use, the captain preferred to land on runway 18,
- The aircraft was unstable on short final and landed too far down the runway, reducing the landing distance available,
- The aircraft' speed upon landing was excessive, preventing the reverse thrust systems to be activated,
- The captain took over control and activated the reverse thrust system on the right engine only,
- Poor crew coordination,
- The crew was operating in a conflict environment after touchdown,
- Poor judgment of the situation,
- Poor flight planning,
- Lack of crew discipline.
