Circumstances:
En route from Bandung to Malang, the twin engine airplane was shot down by the pilot of a Japanese Mitsubishi Zero and crashed into the sea off Surabaya. Two crew members were rescued while five other occupants were killed. The next day, both survivors died from injuries sustained.

Probable cause:
According to Dutch authorities, the pilots thought to splash down on a lake while they put their machine on flooded fields.

After landing at Darmo, the airplane suffered technical problems with the braking systems. It went out of control and came to rest nearby the runway. There were no injuries among the occupants but the airplane was damaged beyond repair due to serious damages to wings and fuselage. The exact date of the occurrence remains unknown (December 1941).
