Circumstances:
The airplane was returning to base following a night Bullseye exercise. On approach, it suffered an engine failure and crashed. Four crew members were killed and three others were injured.

Probable cause:
The starboard inner engine failed in flight and the pilot was unable to contact RAF Wickenby by radio. He was forced to hold whilst the airfield flarepath was illuminated and this was considered as a contributory factor.
