Circumstances:
The crew was engaged in a local test flight at La Carlota-General Francisco de Miranda AFB in Caracas. During the takeoff roll, a tire burst on the right main gear that collapsed and was torn off. The airplane veered off runway to the left then the left main gear collapsed as well and the airplane came to rest on its belly with the nose gear still extended. There were no injuries among the crew.

Aircraft left Valle de la Pascua at 0646LT to La Carlota Air Base, near Caracas. At 0722LT, crew informed ATC he was flying at 7,000 feet, this was the last contact. Aircraft struck a mountain and all three occupants were killed.

After landing on runway 28R, aircraft deviated from its course to the left. It eventually veered off runway to the left and went down an embankment. Nose gear collapsed and aircraft came to rest in overhang. All occupants evacuated safely and aircraft was damaged beyond repair.

Aircraft was on a post maintenance delivery mission. A crew of two engineers was positioning the aircraft from a technical hangar in Caracas-Simon Bolivar Airport to the main terminal. On the ramp, aircraft became out of control, went into the grass and collided with a drainage ditch, causing the left main gear to collapse and the left wing to be severely damaged. Both crew were uninjured while the aircraft was considered as damaged beyond repair.
Probable cause:
Loss of control for undetermined reasons.

Enroute, while descending from 6,700 to 5,000 feet, the aircraft crashed in a mountainous terrain located 29 km southwest of Caracas. Rescue teams arrived on the crash site in the morning of 7 May. One crew member was seriously injured while all other occupant were fatally injured. The only survivor died few days later.
