Circumstances:
Following an uneventful flight from Manaus, the crew started the descent to Rio Branco-Presidente Médici Airport runway 06 in marginal weather conditions with limited visibility due to fog. After passing the MDA at 1,150 feet, the crew continued the approach with an excessive rate of descent of 1,222 feet per minute. At a height of 57 feet, the aircraft struck trees located 360 metres short of runway 06 threshold. The engines lost power, the aircraft lost speed and height then struck the ground about 100 metres short of runway. Upon impact, the undercarriage were torn off and the aircraft slid on a distance of 600 metres before coming to rest. All 95 occupants escaped uninjured while the aircraft was destroyed.
Probable cause:
Poor approach configuration on part of the flying crew who continued the approach after passing the MDA at 1,150 feet while the aircraft was not stabilized and the crew did not establish a visual contact with the obstacles on short final. The following contributing factors were identified:
- Following the first impact with trees, branches and pieces of wood entered the engines, causing a loss of power followed by a loss of speed and altitude,
- Complacency on the part of the crew,
- Pressure and stress,
- Visual perception issues,
- Violation of the standard operating procedures,
- Loss of situational awareness,
- Lack of crew resources management,
- Poor approach and landing planning,
- Poor weather conditions,
- Relative low experience of the copilot.

Circumstances:
Following a night takeoff from São Paulo-Guarulhos Airport, en route to Buenos Aires, the captain informed ATC about technical problems and was cleared to return for an emergency landing. The aircraft landed at a speed of 185 knots with flaps down to 15° on wet runway 09L. Unable to stop within the remaining distance, the aircraft overran, lost its undercarriage and came to rest 200 metres further. All 128 occupants were evacuated safely, among them two passengers were slightly injured. The aircraft was written off.
Probable cause:
It was determined that the n°3 leading edge flap actuator attachment fitting on the wing front spar had fractured due to corrosion. The actuator came away and caused the failure of some hydraulic lines and damage to the thrust control cables. Some 1981 Boeing Service Bulletins had not been complied with. One of these included the replacement of the aluminium leading edge flap actuator attachment fitting with a steel one; this had not been done. The following contributing factors were reported:
- Excessive workload on approach and landing due to the emergency situation,
- Poor approach planning,
- Lack of visibility due to the night,
- Poor crew coordination,
- Poor crew resources management,
- The crew forgot to lower the flaps electrically on approach, causing the speed to be 32 knots in excess,
- Poor aircraft maintenance and supervision,
- Failures in the supervision of the Company's operating sector.

Circumstances:
While descending to Cruzeiro do Sul Airport by night and good weather conditions, the crew encountered problems with the intermittent activation of a warning light in the instrument panel, warning them of a fire in the cargo compartment. On final approach, the aircraft struck trees and crashed in a dense wooded area located in hilly terrain. The wreckage was found 15 km from runway 10 threshold and all three occupants were killed.
Probable cause:
The following findings were reported:
a. Human Factor
(1) Physiological Aspect
- There was no evidence of this aspect contributing to the occurrence of the accident.
(2) Psychological Aspect - Contributed
- The psychological aspect contributed through the generation of a high level of anxiety to perform the landing and in the diversion of the focus of attention during the approach manoeuvres to land.
- The psychological aspect was influenced by the activation of the smoke alarm which generated an increase in the workload on board.
b. Material Factor
- There were no indications that this factor contributed to the accident.
c. Operational Factor
(1) Deficient Instruction
- Although the instruction was carried out in accordance with what the standards recommend, the failures that contributed to the accident are characteristic of lack of experience in facing abnormalities simultaneously with the maintenance of flight control. Such failures could be avoided with more adequate simulator instructions and training involving the cockpit management aspects.
(2) Poor Maintenance - Undetermined .
- It was not possible to determine the cause of the activation of the 'Aft Cargo Smoke' alarm and whether the maintenance services contributed to this occurrence.
(3) Deficient Cockpit Coordination
- Inadequate performance of the duties assigned to each crew member. The procedures foreseen for the execution of descent by instrument have been modified and some have been deleted depending on the appearance of a complicator element (smoke alarm).
(4) Influence of the environment
- The dark night contributed to the creation of the 'black hole' phenomenon, or 'background figure', making it difficult to perceive external references for a possible identification of the vertical distance of the aircraft from the ground.
(5) Deficient Oversight
- The supervision, at cockpit level, contributed to the accident by the inadequate management of the resources available for the flight in the cockpit.
- Company level supervision contributed to the accident by not identifying the need for cockpit management training and providing it to the crew involved.
- Supervision, at company level, was also inadequate when climbing to the same mission, two pilots unfamiliar with the airplane to be used and in night operation.
(6) Other Operational Aspects
- The failure to comply with several 'Callouts', the non-use of the radio altimeter and the inadequate use of the 'altitude alert', as an aid to the accomplishment of the descent procedure, contributed to the occurrence of the accident.

Circumstances:
The crew left the apron and taxied to the runway threshold for takeoff on a regular schedule service VP210 to Belo Horizonte. Due to foggy conditions, the visibility was nil and the crew mistakenly lined up on the 'threshold' of the taxiway instead of the runway. Unaware of the wrong position of the aircraft, the crew was cleared for takeoff by ATC and started the takeoff procedure. After few seconds, the captain realize the situation and initiated an emergency braking maneuver. Unable to stop within the remaining distance, the aircraft overran, struck an embankment and came to rest, broken in two. A passenger was killed, 20 other occupants were injured and the aircraft was destroyed.
Probable cause:
Failure of the crew to recognize that he was lined up on a taxiway and not the active runway. Poor visibility due to foggy conditions was a contributing factor.

Circumstances:
The crew (one pilot and one instructor) was engaged in a local training flight at Manaus-Eduardo Gomes Airport. After a simulated asymmetrical power takeoff, the airplane encountered difficulties to gain sufficient height. The crew increased engine power but the airplane stalled and crashed in a wooded area located few km from the airfield, bursting into flames. Both pilots were killed. It is believed that the aircraft was on its way to São Paulo Airport, carrying a load of televisions.
Probable cause:
The accident was the consequence of the combination of the following findings:
- Poor flight preparation,
- Lack of crew coordination,
- Failure to follow pre-takeoff procedures,
- Inadequate supervision on part of the instructor.

Circumstances:
Following an uneventful flight from Rio de Janeiro-Galeão Airport, the crew initiated a night descent to Fortaleza-Pinto Martins Airport, Ceará. ATC cleared the flight to descent to 5,000 feet and while facing the city lights, the captain descended below the prescribed altitude of 5,000 feet. Despite two altitude alert system warnings and the copilot's warning about mountains ahead, the captain continued to descend until the aircraft struck a wooded mountain located in the Aratanha Mountain Range, some 26 km southwest of the airport. The aircraft disintegrated on impact and all 137 occupants were killed.
Probable cause:
For unknown reasons, the captain continued the descent below 5,000 feet as instructed by ATC and ignored GPWS and pilot warnings.

The approach to Brasília was initiated in poor visibility due to rain falls. On short final, the crew failed to realize that the aircraft attitude was wrong. In a nose-down attitude, the aircraft landed hard, causing the nose gear to collapse. The airplane went out of control, veered of runway and came to rest, broken in two. Two passengers were killed, 20 occupants were injured and 96 others escaped uninjured.
Probable cause:
It is believed that the crew suffered an optical illusion on short final.

Following an uneventful flight, the crew was unable to lower the undercarriage and decided to complete a belly landing. Upon touchdown, the airplane slid for few hundred meters before coming to rest in flames onto the runway. All 44 occupants were evacuated safely while the aircraft was damaged beyond repair.

Shortly after takeoff from runway 34 at São Paulo-Congonhas Airport, while climbing, the crew reported problems with the right engine and attempted to return for an emergency landing. Shortly later, the airplane went out of control and crashed in flames onto several houses located in the district of Campo Belo near the airport. The aircraft and four houses were totally destroyed and all 15 occupants were killed. There were no victims on the ground.
Probable cause:
Failure of the right engine after takeoff for unknown reason.
