Circumstances:
The aircraft took off from Pickle Lake, Ontario on a flight to Round Lake, Bearskin Lake, Big Trout Lake and Big Beaver House. The type of flight and time of departure were not stated in the report. The aircraft did not arrive at any of the intermediate points nor at its destination. Nothing further was heard or seen of it until it was found on 30 May 1963 approximately 73 miles northwest of Pickle Lake (52° 24'N - 90°54'W). The investigation showed that break up of the left wing had occurred in flight prior to impact. The time of the accident was calculated to be 1215LT. The pilot was killed and the aircraft was destroyed.
Probable cause:
The accident was attributed to fatigue failure of the front eyebolt on the left wing strut.

Shortly after takeoff from Sandy Lake, the single engine aircraft stalled and crashed in a wooded area. The aircraft was destroyed and all eight occupants were killed. It was reported that a non-authorized person was sitting in the left seat in the cockpit at the time of the accident.

Circumstances:
The aircraft departed Rankin Inlet at 1440LT on a non-scheduled flight to Chesterfield Inlet with a pilot, three passengers and freight on board. At 1455LT low clouds and airframe icing were encountered. The pilot altered the aircraft's heading toward the coastline of Hudson Bay and when over the coastline, turned left in an attempt to reach Chesterfleld Inlet. However, the pilot decided to land the aircraft at the first opportunity and, at about 1505LT, struck the snow-covered ground in whiteout conditions. The wreckage was found by an RCMP constable on his annual patrol about 30 minutes after the accident occurred. The aircraft was demolished and the four occupants were seriously injured.
Probable cause:
The pilot continued VFR flight into unfavorable weather conditions and attempting to land, collided with the ground.
