Circumstances:
At 1834 Pacific standard time, the Nav Air Charter Inc. Mitsubishi MU-2B-36 aircraft (registration C-FTWO, serial number 672) took off from Runway 15 at the Terrace Airport for a courier flight to Vancouver, British Columbia. The left engine lost power shortly after take-off. The aircraft descended, with a slight left bank, into trees and crashed about 1600 feet east of the departure end of Runway 15 on a heading of 072° magnetic. The aircraft was destroyed by the impact and a post-crash fire, and the two pilots were fatally injured.
Probable cause:
Findings as to Causes and Contributing Factors:
1. During the take-off, the left engine combustion chamber plenum split open due to a fatigue crack. The rupture was so extensive that the engine flamed out.
2. The crew did not feather the left engine or retract the flaps, and the aircraft entered a moderate left-hand turn after take-off; the resulting drag caused the aircraft to descend until it contacted trees.
3. The first officer’s flying skills may have been challenged during the handling of the engine failure, and the checklist was conducted out of sequence, suggesting that there may have been uncertainty in the cockpit. A contributing factor may have been the captain’s unfamiliarity with handling an emergency from the right seat.
4. The use of flap 20 for take-off, although in accordance with company policy, contributed to the difficulty in handling the aircraft during the emergency.
Findings as to Risk:
1. The TPE331 series engine plenum is prone to developing cracks at bosses, particularly in areas where two bosses are in close proximity and a reinforcing weld has been made. Cracks that develop in this area cannot necessarily be detected by visual inspections or even by fluorescent dye-penetrant inspections (FPIs).
2. Because the wing was wet and the air temperature was at 0°C, it is possible that ice may have formed on top of the wing during the take-off, degrading the wing’s ability to generate lift.
3. Being required to conduct only flap 20 take-offs increases the risk of an accident in the event of an engine problem immediately after take-off.
Other Finding:
1. The plenum manufactured with a single machined casting, incorporating the P3 and bleed air bosses, is an improvement over the non-single casting boss plenum; however, cracks may still develop at bosses elsewhere on the plenum.

Circumstances:
The aircraft was on a scheduled cargo flight from Nanaimo, British Columbia, to the civilian terminal on the south side of the military airbase at Comox, British Columbia. The crew members established communication with the Comox tower when they were at about 2000 feet over Hornby Island, 12 nautical miles southeast of Comox, and requested a practice back course/localizer approach to Runway 30, circling for landing on Runway 18. The request was approved and the aircraft continued inbound. When the aircraft was about two miles from the threshold of Runway 30, the crew declared an emergency for an engine fire in the right engine. The tower alerted the airport response teams and requested standard data from the crew concerning the number of people and amount of fuel on board. Less than 30 seconds after the crew first reported the emergency, the aircraft was engulfed in flames. Shortly thereafter, at 0741 Pacific daylight time, the aircraft rolled inverted and struck the ground in a steep, nose-down, left-wing-low attitude. The aircraft broke apart and burned. Both crew members were fatally injured.
Probable cause:
Findings as to Causes and Contributing Factors:
1. At some point after 01 April 1999, a bad gasket (P/N LW-13388) was installed in the accident engine.
2. The requirement of Airworthiness Directive 2002-12-07 (to ensure that old converter plate gaskets were removed and replaced by new parts) was not carried out on the accident engine.
3. The improper oil filter converter plate gasket in the right engine compartment failed, allowing pressurized oil to spray into the engine compartment and ignite on contact with hot turbocharger and exhaust components.
4. The firewall fuel shut-off valve remained in the OPEN position, allowing pressurized fuel to be delivered to the engine-driven fuel pump by the aircraft’s boost pumps.
5. The initial oil-fed fire generated considerable heat, which melted the casing of the engine-driven fuel pump, allowing pressurized fuel to intensify the fire.
6. The flames breached the main fuel tank, inboard of the engine, causing the aircraft to become engulfed in flames.
Findings as to Risk:
1. Inappropriate converter plate gaskets, identified by part number LW-13388, are known to have remained in the aviation system after the date of the terminating action required by Airworthiness Directive (AD) 2002-12-07.
2. Compliance with the full requirements of AD 2002-12-07 is not always being accomplished with respect to vibro-peening and proper gluing procedures.

Circumstances:
The Navair Charter Piper PA-31, flight FCV705, a mail courier with a crew of two, was on a night, instrument flight rules (IFR) flight plan from Williams Lake, British Columbia, to Kamloops. While the crew was conducting an instrument procedure to the Kamloops airport, they flew into an area that was clear of cloud. The captain cancelled his IFR flight plan and descended for a visual flight rules (VFR) approach to the airport. At 1902 PST, the aircraft collided with trees at an elevation of 2,400 feet above sea level (asl), seven nautical miles (nm) east-southeast of the Kamloops airport. The captain was fatally injured on impact, and the first officer was seriously injured; the aircraft was destroyed during the crash and post-crash fire.
Probable cause:
The aircraft collided with trees during a night, VFR descent over mountainous terrain because the crew had inadequate visual cues to accurately determine their height above the ground. Contributing to this accident were the captain's decision to cancel his IFR flight plan and attempt a visual approach from well outside the published VFR circuit area, the inherent risks of VFR flight in mountainous regions, and a requirement by the crew to remain clear of departing IFR traffic from the Kamloops airport.
