Circumstances:
The airplane departed Brasilía-Presidente Juscelino Kubitschek Airport in the evening on a charter flight to São Paulo-Congonhas, carrying two pilots and one passenger, the Senator Aécio Neves da Cunha. During the takeoff roll, a tire on one of the main landing gear failed. The crew continued the flight, informed ATC about his situation and preferred to divert to São Paulo-Guarulhos Airport that offered longer runway for an emergency landing. After touchdown by night, the aircraft deviated to the right then veered off runway. The left main gear collapsed and the aircraft came to rest in a grassy area. All three occupants evacuated safely and the aircraft was damaged beyond repair.

Circumstances:
On April 3, 2012, about 0920 atlantic standard time (ast), a Hawker Beechcraft C90GTx, N8116L, operated by Lider Taxi Aereo, was substantially damaged after ditching in the waters of the Caribbean Sea, 17 miles north of Aruba, following a dual loss of engine power during cruise. The flight departed Fort Lauderdale Executive Airport (FXE), Fort Lauderdale, Florida, and was destined for Hato International Airport (TNCC), Willemstad, Curacao. The airline transport pilot and the pilot rated passenger were uninjured. Visual meteorological conditions prevailed, and an instrument flight plan was filed for the delivery flight conducted under 14 Code of Federal Regulations Part 91. The Amsterdam arrived at the ditching location at 1120. The airplane was partially submerged. The crew of the Amsterdam attempted to prevent the airplane from sinking by placing a cable around it and hoisting it onboard. However during the attempted recovery, the fuselage broke in half and the airplane sank.
Probable cause:
Review of the fuel ticket revealed that the misspelled words; "Top Neclles" was handwritten on it. It was also signed by the pilot. Further review revealed that only 25 gallons had been uploaded to the airplane, and this number had been entered in the box labeled "TOTAL GALLONS DELIVERED". Review of the start reading and end reading from the truck meter also concurred with this amount. Furthermore, It was discovered that the "134 gallons" that the pilot believed had been uploaded to the airplane was in fact the employee number of the fueler that had topped off the nacelle tanks and had entered his employee number on the "FUEL DEL BY:" line. Utilizing the information contained on the fuel ticket, it was determined that the airplane had departed with only 261 gallons of fuel on-board. Review of performance data in the POH/AFM revealed that in order to complete the flight the airplane would have needed to depart with 328 gallons on-board.

Circumstances:
The aircraft was completing an ambulance flight from Palm Beach to Rio de Janeiro with intermediate stops in Aguadilla and Manaus, carrying one patient, a medical team and two pilots. During the takeoff roll from 10 at Manaus-Eduardo Gomes Airport, just before V1 speed, the crew heard a loud noise coming from the right side of the airplane. In the mean time, the aircraft started to deviate to the right. The captain decided to abandon the takeoff procedure and initiated a braking maneuver. Unable to stop within the remaining distance, the aircraft overran and came to rest 400 metres past the runway end. All six occupants escaped uninjured while the aircraft was damage beyond repair.
Probable cause:
The following findings were identified:
- The external tyre on the right main gear deflated during the takeoff roll,
- The crew retarded the power levers and deployed the spoilers,
- The crew did not use the parachute, judging the relative low speed and thinking this was an optional equipment,
- Technical analysis on the right main gear revealed that the six bolts on the external wheel torque were approximately 90% lower than foreseen, which may contributed to the tyre deflection.

Circumstances:
The crew departed São Paulo-Congonhas Airport on a positioning flight to São José dos Campos. While descending to São José dos Campos, the captain led the controls to the copilot who was still under instruction. On final, the aircraft was too high on the glide. The captain took over controls but his reaction was excessive. The aircraft suddenly rolled to the right, causing the right wing to struck the ground few dozen metres short of runway 15 threshold. The aircraft landed and came to rest on the main runway. Both pilots evacuated safely while the aircraft was damaged beyond repair.
Probable cause:
The captain did not conduct a preflight briefing and then improvised during the descent by deciding to leave the controls to the copilot while he was still under instruction.
The following contributing factors were identified:
- The copilot who was pilot-in-command on final was in his initial training process,
- The captain authorized the copilot to be the PIC while he was still under initial training,
- The captain was not qualified to operate as an instructor,
- The captain did not make any simulator training for more than two years,
- The copilot had never completed any simulator training since the beginning of his training,
- Lack of crew coordination,
- Poor judgment on part of the captain.

The aircraft departed São Paulo-Congonhas Airport on an ambulance flight to Uberlândia, carrying two doctors and two pilots who should pick-up a patient. The copilot was pilot flying. This was his first flight on a Learjet without an instructor. This was against regulations since the copilot was not fully qualified yet and the captain was not qualified to act as an instructor. Upon arrival at Uberlândia, weather conditions were poor with a low ceiling at 100 meters (clouds 8/8), fog and an horizontal visibility less than 200 meters (below minimums weather conditions). The crew abandoned the approach and initiated a go-around procedure. Few minutes later, a second approach was also abandoned. Rather than diverting to the alternate airport, the crew attempted a third approach when the aircraft struck the ground and crashed short of runway threshold. All four occupants were seriously injured and the aircraft was destroyed.
Probable cause:
The accident was the consequence of the combination of the following factors:
- Poor flight preparation and planning,
- Poor approach planning,
- Wrong approach configuration,
- Poor crew coordination,
- Lack of visibility due to the night associated to below minimums weather conditions,
- The crew failed to initiate a go-around and to divert to the alternate airport,
- Poor crew resources management,
- Non observation of operational procedures,
- Lack of supervision on part of the captain,
- Poor operational organization on part of the operator.
