Circumstances:
After takeoff from Musiara Airport, during initial climb, the crew raised the landing gear and flaps. About 45 seconds after liftoff, the left engine failed. The crew elected to restart it but without success and decided to divert to Olkiombo Airport located 18 km from their position. On approach to runway 09, the aircraft stalled and crashed in an open field. The copilot was killed while 19 other occupants were injured, some seriously. The aircraft was destroyed.
Probable cause:
Failure of the left engine during initial climb for unknown reasons.

After landing on a poor condition runway, the aircraft deviated to the left then veered off runway. The aircraft lost its landing gear and came to rest few dozen metres further. All 10 occupants escaped injured and the aircraft was written off. The accident occurred at Mogadishu-South Airport sometimes called Mogadishu-West Airfield or KM50. Its narrow and limited runway is made up of chippings and pebbles and is not equipped with a lighting system or any drainage system and is very poorly maintained.

En route from Mombasa to Kichwa Tembo, the pilot was informed that a DC-3 suffered a mishap upon landing at Musiara Airstrip and apparently decided to make a low pass to show the scene to the passengers. While approaching the airfield at an altitude of 250 feet, the aircraft collided with a white-backed vulture who went through the windshield and killed the pilot instantly. Control was lost and the aircraft crashed in an open field, bursting into flames. All nine occupants were killed.
Probable cause:
Loss of control following an in-flight collision with a white-backed vulture weighing 5,4 kilos that struck the windshield and killed the pilot.
