Circumstances:
The crew as engaged in a circular training flight in Mallala. Shortly after take off, the twin engine aircraft collided with a second RAAF Avro 652 Anson registered A4-6 with two pilots on board. Both aircraft went out of control and crashed near the airport. All four crew members were killed. The exact circumstances of the mishap remains unknown.
