Circumstances:
During initial climb, the crew encountered difficulties to gain height. The crew elected to make an emergency landing when the aircraft crashed in an open field some three minutes after takeoff. The wreckage was found 500 metres from the runway end. Four occupants escaped uninjured while 11 others were injured. The aircraft was written off.
Probable cause:
It was determined that the aircraft was overloaded.

Circumstances:
Following an uneventful flight from Kotlas, the crew started the descent to Arkhangelsk-Talagi Airport. On approach at an altitude of 600 meters, the crew was instructed by ATC to hurry the last and third turn. This caused hasty actions of the crew. The captain asked the flight engineer to reduce power on all three engine to idle but once the altitude of 580 meters was reached on descent, the flight engineer mistakenly shut down all three engines. The pilot tried to restart them but without success because the approach speed was too low. The captain attempted an emergency landing on a road. Upon touchdown, the airplane collided with the support of a power transmission tower and came to rest. All 27 occupants were rescued, among them 23 were injured.
Probable cause:
Erroneous actions on part of the flight engineer who put the power control levers in the 'stop' position instead of the 'idle' position. The lack of proper control on part of the both pilot and copilot was considered as a contributing factor.

The airplane was completing a ferry flight from Kotlas to Voronezh with two passengers and a crew of five on board. The flight was operated at an altitude of 1,500 meters from Kotlas to Kirov and after passing Kirov, the crew was cleared to climb to 1,800 meters. At 1056, the crew was instructed for heading 219° when nine minutes later, the airplane entered a dive and crashed in an uninhabited area located northwest of Sovetsk. The wreckage was found in the afternoon. The aircraft was destroyed by impact forces and a post crash fire and all seven occupants were killed.
Probable cause:
It was determined that the aircraft became uncontrollable after it encountered severe turbulences while cruising in thunderstorm activity. Investigations revealed that the crew has not been informed about the possible presence of thunderstorms as the main weather radar of the local meteorologist was out of service due to maintenance.

During the takeoff roll at Arkhangelsk-Talagi Airport, at a speed of 160 km/h, the captain started the rotation but the airplane failed to rotate. The pilot failed to abort and asked the copilot to detect the reason of the problem when, after a course of 2,250 meters, the airplane overran the runway. It rolled in bushes on a distance of 314 meters then struck the localizer antenna and a small building located 116 meters further. Out of control, the airplane continued on a distance of 55 meters and collided with a concrete pillar before coming to rest in flames. The copilot was killed, the captain and the flight engineer as well as 11 passengers were injured, the others 17 occupants were unhurt. The aircraft was destroyed by impact forces and a post crash fire.
Probable cause:
It was determined that an electrical problem from an unknown source occurred on the elevators locking system, causing the elevators to be jammed during the takeoff procedure. The reaction of the captain was inadequate as he failed to abandon the takeoff manoeuvre when the problem was detected.

After takeoff from Kotlas, at a distance of 40 km from his departure point, the pilot entered an area of thunderstorm activity. Rather than to return, he decided to continue when lightning stroke around him. Afraid and disoriented, the pilot cut off the contact and elected to make an emergency landing but finally decided to continue. As the contact was off, the magneto was not functioning so the engine failed to respond. The airplane crash landed in a field and collided with trees. Both occupants were uninjured.

The airplane was completing a schedule service from Arkhangelsk to Syktyvkar with an intermediate stop in Kotlas, carrying one passenger and two crew members. The crew departed Kotlas at 1240LT on the final leg to Syktyvkar. Approaching the village of Korovinskaya, the pilot encountered sudden heavy snow falls for a period of time of about 15-20 minutes. The visibility was very poor and the pilot decided not to return and continued in such conditions. Few minutes later, while cruising in a flat attitude in white conditions, the airplane impacted ground and crashed one km southeast of Slobodshikovo. The aircraft was destroyed and all three occupants were killed.
Probable cause:
Controlled flight into terrain following the pilot's decision to continue the flight at an insufficient altitude in reduced visibility due to heavy snow falls while it would have been better to fly back to Kotlas.
